Communication aids and language assistance

These services are free. You will never be charged for them, and you do not need to bring your own interpreter.

We provide aids and services to people with disabilities so they can communicate effectively with us, such as:

• Written information in other formats, including large print, audio, and accessible electronic formats

We provide language services to people whose primary language is not English, such as:

• Qualified interpreters
• Information written in other languages

If you believe we have discriminated

If you believe Serene Medical Spa LLC has failed to provide these services, or has discriminated against you in any other way, please tell us. We take these concerns seriously and will look into them.

You can raise a concern in person, by mail, by phone, or by email. If you need help putting it in writing, we will assist you. We will not retaliate against you for raising a concern.

Filing a complaint with a state agency

You may also file a complaint directly with the civil rights agency in the state where the treatment took place. You do not need a lawyer, and you do not need to contact us first.
